Care coordination model: better care at lower cost for people with multiple health and social needs

This report is part of the Innovation Series 2011 published by the Institute for Healthcare Improvement. This white paper outlines methods and opportunities to better coordinate care with people with multiple health and social needs, and reviews ways that organizations have allocated resources to better meet the range of needs in this population. There is special emphasis on the experience of care coordination with populations of people experiencing homelessness. Discussion includes measures used to track the impact of these efforts on health, costs, and experience of care.

Contracting for personalised outcomes: learning from emerging practice

This report draws on the learning from six councils and shows how they have begun to change their approach to contracting, service development and provider relationships to be more compatible with the aims of personalisation and personal budgets. It provides a summary of the main components of the contractual models identified (personal budgets, service personalisation and outcomes focused framework contracts), a framework for understanding the relationship between them and a brief account of the key messages from the case studies.

Working together to provide age-appropriate environments and services for mental health patients aged under 18

The new section 131A of the Mental Health Act is due to come into force in April 2010. This amendment will ensure that patients aged under 18 are treated in an environment in hospital which is suitable having regard to their age, with the purpose of preventing the inappropriate admission of children and young people to adult psychiatric wards. This briefing highlights how commissioners can work together to meet the new duty on age-appropriate accommodation in a timely manner.

Guide outlining how the 18 weeks referral to treatment standard applies to non-emergency consultant-led CAMHS services and pathways in England. It also demonstrates how accessible low-wait CAMHS can be achieved and provides examples of good practice.
