Nine local authority led partnerships have been selected as phase 1 of the Common Assessment Framework for Adults (CAF) Demonstrator Site Programme. The programme aims to develop and test improved information sharing across health, social care and wider community support services.
This document contains eight case examples focusing on different aspects of information sharing. These eight case examples support the cross-Government guidance document Information Sharing: Practitioners’ Guide by illustrating for practitioners the practical application of the guidance. The case examples cover a range of situations of relevance to everyone who work with children and young people, whether they are employed or volunteers, working in the public, private or voluntary sectors.
Paper focusing on information about the quality of health services and looking at ways in which it can lead to better quality healthcare for all. It argues that the information needed to improve services is not always available and that a deeper level of information along with measures such as patient-based outcome measures are required to change health care for the better.
